PDF invoice is created not from template but from file "invoice_functions.php". You need to edit that file to change the layout of teh PDF invoice. This is the answer to both your questions.
This is a bit inconsistant, would be better PDF and HTML invoices are generated from the same base template.

I tried to change the logo image in the invoice and packing slip and get an error now. Why? What was the original logo pathname so I can change it back?

I checked the demo admin, and it shows the path to the logo is ../images/silver/logo.jpg Maybe that will lead you in the right direction for your installation.
