Hi I guess this is probably looking me in the face , but I want to set up a simple how did you find us form. I would give several possibilities advert word of mouth etc etc..so that the customer could select it. This is different to a poll as i want the result to appear ideally on all the paperwork ie email or invoice ..I could then enter this on their record. Any suggestions would be most welcome! I collect this information when the customer signs up. Alternatively, you could collect this information on the order screens as well.
 
Just add custom fields, with a drop down box.
 
For example:
 
http://www.mysite.com/admin/admin_user_profile.php?type_id=1
 
Then click on Custom Fields and create a field of choice as I did n the attachment images!

I have been using the poll option but i don't think it saves the info for the particular customer or if so that its exportable into one of the paper receipts ie emails or order pdfs.

No a problem... remember, you can always hook up excel to your database too in order to build reports off of your site...
