Today I turned friendly urls on, following the directions and it worked fine. However I was testing a downloadable product and when I added it to the cart was sent this error with a nearly blank cart page.
Notice: Undefined variable: page_friendly_url in /home/tidalwav/public_html/blocks/block_products_related.php on line 175
Support solved it, I used wrong product promotion in the cart.

I will copy here the answer from support ticket in case somebody encounters the same problem.

It looks like you're using the wrong block. "Products Related" works only on product's details page. We recommend to use this one instead "Shopping Cart: Recommended Products". It shows products from Related and Accessories sections of the added to cart products.
